A Patient-Centered Medical Home (PCMH) is a team-based model of health care in which a primary care provider manages and coordinates a patient’s care on a continuous basis for optimal health results.

A PCMH practice provides all of a patient’s healthcare needs and arranges with other healthcare professionals when necessary.

Both you, the patient, and we, the healthcare providers have specific roles and responsibilities, which we outline below.

Our Responsibility

When you visit us, your primary care provider will learn about you, your family, your life situation, and health goals and preferences. We will remember these and your health history every time you seek care and we will suggest treatments that make sense for you. The care we deliver will be based on your unique needs and circumstance.

We will take care of any short-term illness, long-term chronic disease, and your all-around well-being. Additionally, we will keep you up-to-date on all your vaccines and preventive screening tests and help you guard against conditions that impact your health.

As your primary care provider, we will connect you with other members of your care team (this includes specialists and health coaches) and coordinate your care with them in order to get the best results for you. If you have pressing issues to be addressed, we will be available after hours for any urgent needs.

Your healthcare provider, be it a family nurse practitioner or another member of our team may recommend some medical tests to aid in diagnosis. We pledge to notify you of test results in a timely manner. As we understand the value of effective communication in promoting good health, we will communicate any results or details in a timely manner. We also pledge to clearly communicate with you so that  you understand your condition(s) and all your options which will enable you to make informed choices about your health.

Beyond diagnostics and treatment, our primary care providers will listen to your questions and feelings and understand you if you feel overwhelmed by any medical conditions affecting you. We will address your concerns about your condition and treatment. When you reach out to us, we will respond promptly to you – and your calls – in a way you understand.

Our aim is to provide you with high quality healthcare — our goals include both educating you about your health conditions and help you both learn and understand any illnesses while educating you on how to manage your own condition.

Your Responsibility

We trust you to know that you are a full partner with us in your care. We invite you to share your feedback, concerns, and preferences with us. When meeting our family nurse practitioner, we expect you to come to each visit with any updates on medications, dietary supplements, or remedies you’re using, and questions you may have. Your cooperation will help us to determine the right plan for you.

You may need to see different qualified professionals for various reasons. Let us know when you see other health care providers so we can help coordinate the best care for you by updating them with your information. Help us by keeping your scheduled appointments or calling to reschedule or cancel as early as possible. Maintaining appointment times will help us serve you better.

You have an important role to play in your care. It will help if you understand your health condition: ask questions about your care and tell us when you don’t understand something. We will always address your concerns and questions. Learn about your condition(s) and what you can do to stay as healthy as possible. Knowledge of your health status will help you live a healthy life.

When we work together on your health, follow the plan that we have agreed is best for your health. We expect you to take medications as prescribed in order to get the expected medical results. Do note that following a different path may have negative consequences.

You can always access your health information at any time — you can use our patient portal to track your health and to share your personal health record when appropriate with other caregivers. Results sometimes take time to arrive — however please do not hesitate to reach out to us if you do not receive your test results within two weeks.

We understand you may need us outside of business hours for serious issues and we make arrangements to help in emergencies. We ask that you contact us after hours only if your issue cannot wait until the next work day. Emergencies happen unexpectedly. If possible, contact us before going to the emergency room so someone who knows your medical history can care for you.
